INTRODUCTION:

I love paperwork!  Lots of it!  Does this sound like you?  No? Me either.  Clinical documentation is considered the necessary evil in our industry. Some of the things we have been taught over the years include:  Keep your notes vague! If you don’t accept insurance, your notes don’t really matter. You never know when your notes could be used against a client, so always keep them generalized. Our documentation, regardless of whether we’re in private practice or at an agency, are legal documents, and they need to reflect the same accuracy and detail expected of medical professionals.  And now that Covid has changed the way we deliver services, how do we document TeleHealth Sessions? Does it affect your style? Feel like it’s time for a refresher?

LEARNING OBJECTIVES INCLUDE:

Define medical necessity and learn how to document it effectively

Define concurrent documentation and discuss the rationale of why it should be utilized with clients

Identify potential red flags in clinical documentation and the proper corrective measures

Learn the key components of creating appropriate progress notes specifically for telemental health sessions
